To open a mailbox you will need to fill out two forms, psForm 1583 and a Mailbox Rental Agreement. Form 1583 MUST be notarized. Originals must be mailed to us, along with a clear photocopy of Drivers License and a second Form of ID (acceptable forms are listed in the lower portion of box #8). Any person receiving mail to the mail box that is 18 years of age or older must send a copy of Drivers License. To begin you will need to print out the forms

Please remember that we can't open your mailbox until the Mailbox Rental Agreement and the Form 1583 are received. Please fill them out, as explained above, and send them to 8635 W Sahara Ave. Las Vegas NV 89117. Once the forms are recieved we will contact you with box information and setup forwarding times and address information if needed.
